ERISA requires your employer to provide these documents free of charge. Review the terms of coverage and any eligibility requirements you must meet. Another key element in getting your claim approved is to provide thorough documentation of the medical care you receive and the impacts you suffer as a result of your condition. Keeping a long term disability journal allows you to keep track of this and other information pertaining to your case, which can be used both in filing your claim and in appealing denied benefits. 

Information to Include in Your Disability Journal

The Centers for Disease Control and Prevention (CDC) warns that long term disabilities often occur as a result of chronic health conditions. Keep a journal in which you write down information about your condition and the care you receive as soon as you are diagnosed.   • A list of all medical providers;
  • Details regarding treatments and any medications you are taking;
  • Pain, limited mobility, and other symptoms you suffer on a regular basis;
  • The impacts your condition has had on your daily life and that of your family members.
