Plaintiff Ramona Contreras filed this lawsuit under § 502(a)(1)(B) of the Employee Income Security Act of 1974 (“ERISA”), 29 U.S.C. § 1132(a)(1)(B), to recover long-term disability (“LTD”) benefits under a plan that her former employer, Flexible Steel Lacing Company (“Flexco”), established through Defendant United of Omaha Life Insurance Company.
